The following documents are required by new students and transfer students from another school within the system to enroll in Atlanta Public Schools:

  • Georgia Certificate of Immunization
  • Official Certificate of Birth
  • Georgia Certificate of Ear, Eye and Dental Examination
  • Proof of residence of custodial parent or legal guardian:

    1. Deed, Current Mortgage Statement or Lease;
    2. and Electric utility bill w/in 30 days of date of enrollment
  • Student's Social Security Card
  • Withdrawal Form from previous school
  • Transcript and/or report card from previous school address, telephone and fax number of previous school
  • Discipline Report
  • Current Photo ID of parent or legal guardian
  • Proof of guardianship, if not parent (Official Court Order)
  • Administrative Transfer Letter if out of zone
